Support Services Worker – Sault Ste. Marie & Area

(Full-time, One Year Contract with possibility of renewal)

 

Job Purpose:

The aim of the Support Services Worker is to provide individual and family focused care coordination for people living with the effects of acquired brain injury (ABI). The Support Services Worker will work in an ongoing capacity with individuals and families in Sault Ste. Marie and area in an effort to facilitate coordinated holistic health care and recovery journeys. The Support Services Worker will respond to requests for services that have been generated by the hospital, local service providers, community agencies and/or self-referral. The Support Services Worker reports to the Executive Director of OBIA and/or designate.

 

 Primary Duties and Responsibilities:

  1. Individual and Family Advocacy
  • Respond to calls from individuals with brain injury, family members, caregivers, friends and professionals
  • Provide support, advocacy, consultation and conflict resolution on behalf of individuals with ABI and their families who are encountering barriers to accessing appropriate and timely services, funding and resources
  • Provide assistance to individuals living with ABI and their families with completion of applications for social assistance, housing, or applications for service
  • Support individuals with ABI with the transition from hospital to community/home
  1. Service Coordination
  • Assist people with ABI, their families and those who are having difficulty accessing resources by providing support and/or referral.
  • Build and maintain strong relationships and network with ABI providers, hospitals and other related agencies in order to identify the proper channels, locate sources of support and navigate the process to ensure the individual/family gets access to services required for assistance.
  • Serve as the liaison between individuals, their families and a governing system such as hospital, treatment facility, rehabilitation setting or school.
  • Link with hospital discharge planners, case manager, service coordinators and other providers to facilitate early intervention and appropriate placement
  1. Education and Awareness
  • Provide person and family centered ABI education for individuals, caregivers and community organizations to enhance quality of life.
  • Strengthen awareness of OBIA and services provided
  • Increase awareness of ABI in the community to service providers, local agencies, first responders etc.
  1. Other
  • Consistent and thorough program record keeping and statistical reporting
  • Ongoing training and education
  • Other duties as assigned

 

Qualifications:

Education

  • Post-secondary diploma/degree in Health or Social Services or equivalent comprehensive work experience in a related field

Knowledge, Skills and Abilities:

  • Knowledge of acquired brain injury, relevant legislation and its effects on individuals, families and professional systems
  • Cultural competency
  • Experience with individual/family advocacy
  • High level of proficiency in the use of computers including, word processing, internet research, email, social media
  • High level of professionalism and self- awareness
  • Strong counselling, mediation and conflict resolution skills
  • Superior verbal and written communication skills
  • Ability to work independently
  • Creative problem solving skills
  • Current driver’s license and access to an insured vehicle
  • Fluency in English and French preferred
  • Emergency First Aid training/Non Violent Crisis Intervention an asset
  • Criminal Background Check
