Housing Case Manager (Prevention Specialist) (2)

Community Care of St. Catharines & Thorold is currently accepting applications for the position of a full time (35 hours per week) Housing Case Manager (Prevention Specialist) in our Housing Help Centre to support the needs of individuals and families prioritized based upon assessed need and acuity, and who have lost housing or are at risk of losing housing, and work with them on a path to housing stability. The ideal candidate is someone who uses a strengths-based approach, understands that it is always better to work with people and their strengths, and not for them, and is empathetic, professional, non-judgmental, transparent, and creative. This position reports to the Housing Manager and the CEO.

Job Duties & Responsibilities

  • Provide trauma-informed support to individuals who have lost housing or at high risk of losing housing to resolve their housing issues and /or support them in finding and maintaining safe and secure accommodations
  • Travel to see clients in the community, at appointments in the office, or at partner agencies in the region
  • Gather documentation, use motivational interviewing and assertive engagement to establish facts and work to support the client’s strengths and capabilities in housing goals
  • Work to create a formalized collaboration and client pathway with other housing prevention agencies
  • Provide proactive short to medium-term case-management and offer warm referrals to other service providers and supports as appropriate
  • Mediate with landlords, roommates, and others to maintain safe housing options
  • Seek financial assistance when appropriate
  • Assess requests and sign-up clients at high risk of homelessness who want to participate in agency’s short-term money management program; set up and review budgets for accuracy, follow up monthly, work with partner agencies as needed, and support client’s transition to managing their own payments
  • Provide follow up supports to households and landlords
  • Liaise or case conference with paralegals, the legal clinic, social assistance case managers, and other professionals as necessary to support people in their journey to stable housing
  • Connect with landlords and/or property management companies to enquire about housing availability, set-up client appointments, advocate for client
  • Support clients in arranging for unit viewings and applications for housing
  • Attend meetings, conferences and trainings
  • Always represent the agency in a professional manner

Administrative and Organizational Responsibilities

  • Maintain appropriate documentation for statistics and files
  • Develop organized case plans
  • Provide information for funder reports as necessary
  • Maintain client files and update HIFIS database
  • Provide letters for clients

Job Qualifications and Experience

  • Post-secondary degree, diploma or relevant work experience in a related field
  • Experience or education in housing, addictions, mental health, or social services sector
  • Knowledge of Landlord – Tenant legislation or willingness to learn
  • Willingness to train and adapt to emerging best practices
  • Vulnerable sector check and driver’s license

Hours/Schedule: 8:30 – 4:30, Monday to Friday.

Start Date: As soon as possible

Salary: Competitive salary with benefits after 6 months
