$43,000-$45,000 plus Benefits

Position:

Employed by the Niagara Regional Native Centre (NRNC), the Life Long Care Coordinator will be responsible to the policies and procedures in accordance with Niagara Regional Native Centre and under the direct daily supervision of the Executive Director and/or their designates.

Responsibilities:

The Life Long Care Coordinator will be responsible for the development of Community Support Services based on the community needs, such as:

  • Meal programs
  • Transportation to medical appointments
  • Friendly visiting
  • Care-giver support
  • Adult day programs
  • Respite care
  • Shall be responsible to provide support the development of a comprehensive volunteer support circle
  • Will also ensure the development of community support services with particular attention to the needs of Indigenous seniors/elders, the people with disabilities and the chronically ill with special needs

Qualifications:

  • Knowledge and experience in the area of long term care for Indigenous people in Niagara
  • Experience in group facilitation and programming
  • Ability to assess participants needs
  • Knowledge of social barriers and challenges faced by urban Indigenous people
  • Excellent communication skills, both verbal and written
  • Excellent computer skills and database reporting both qualitative and quantitative
  • Strong problem-solving skills and ability to make sound decisions
  • Time management skills with the ability to prioritize a high volume of work
  • Ability to understand and follow oral and written instructions and to complete assigned tasks with minimal direction
  • Excellent program management and planning skills and the ability to successfully multitask
  • Highly skilled in developing relationships with agencies and co-workers
  • Experience working within an Indigenous community-based organization
  • Must have valid driver’s license and reliable vehicle, Stated in cover letter
  • Must have or able to obtain clear criminal records check for the Vulnerable Sector
