{"id":171988,"date":"2025-04-01T14:19:05","date_gmt":"2025-04-01T18:19:05","guid":{"rendered":"https:\/\/www.jobgym.com\/jobboard\/?post_type=job_listing&#038;p=171988"},"modified":"2025-04-11T00:34:39","modified_gmt":"2025-04-11T04:34:39","slug":"youth-in-transition-worker","status":"expired","type":"job_listing","link":"https:\/\/www.jobgym.com\/jobboard\/?post_type=job_listing&p=171988","title":{"rendered":"YOUTH IN TRANSITION WORKER"},"content":{"rendered":"<p><strong>Classification:<\/strong> Regular, Full-time (Temporary &#8211; 6 Month Contract)<br \/>\n<strong>Salary Range:<\/strong> Approx. $26\/hr.<br \/>\n<strong>Hours:<\/strong> 40 Hrs. \/ week (6 months, with possibility of extension)<br \/>\n<strong>Work Location:<\/strong> 1088 Garrison Road, Fort Erie, Ontario<\/p>\n<h6>POSITION SUMMARY:<\/h6>\n<p>The Youth in Transition Worker is responsible for supporting youth in their successful transition out of the child welfare system into adulthood. The YITW will assist youth between the ages of 16 and 24 in developing and pursuing their goals while providing support to the youth in order to identify, access, and navigate adult service systems relevant to their specific needs. The YITW will also support youth to connect to existing supports and resources within their communities including housing supports, education resources, employment services and training (e.g. financial management, household management), income security (e.g First Nation funding agencies, First Nations band administration, Ontario Works, OSAP), health and mental health services and legal services.<\/p>\n<p>YITW\u2019s will meet with youth through person-to-person, voice-to-voice, email, text message and other interactions, which may occur in the community or neighborhood locations or settings. Services are expected to be provided flexibly, at times that meet the needs of youth, and that are consistent with the identified needs and goals of youth.<\/p>\n<p>YITW\u2019s will need to work with local community agencies to both inform them of the YITW program and also become aware of the services available in the community to refer clients. YITW\u2019s will be expected to work closely with their local societies to ensure that eligible youth know about the YITW program, and also to establish a referral process.<\/p>\n<h6><\/h6>\n<h6>QUALIFICATIONS:<\/h6>\n<ul>\n<li>Child and Youth Worker Diploma \/ Accredited college diploma in the Social Services field and 2 years related experience, or an equivalent combination of relevant education in a related field and experience working with Indigenous youth will be considered.<\/li>\n<li>Current vulnerable sector police record check, or ability to obtain one.<\/li>\n<li>Knowledge of indigenous and mainstream community resources, indigenous cultures and understanding of historic context.<\/li>\n<li>Knowledge of and sensitivity to the life experiences and needs of the youth community, particularly extended care and youth leaving the care of a CAS.<\/li>\n<li>Experience with at-risk youth.<\/li>\n<li>Knowledge and skills in the delivery of group programs and workshops that incorporate strength based and evidence based practices. Excellent organizational, oral\/verbal\/written communication skills, excellent computer and technical skills.<\/li>\n<li>Ensuring accuracy, confidentiality and safekeeping of agency records.<\/li>\n<li>Flexibility to be able to work evenings and weekends, if needed.<\/li>\n<li>Flexibility and ability to work within a team approach of service delivery.<\/li>\n<li>Liaise with Mental Health and Child Welfare programs to support transitional aged youth living within the Niagara Region.<\/li>\n<li>Organizing and participating in public education seminars, workshops, groups and training for youth, parents and service providers.<\/li>\n<li>Ensuring that the youth\u2019s rights are respected and addressed in accordance with legislation and NCNW\u2019s Policies and Procedures.<\/li>\n<li>Maintaining service statistics and a system of documentation and reporting that is consistent with the standards and procedures of the service delivery process, legislation and practices of NCNW.<\/li>\n<li>Ensuring all data, forms, reports and necessary documentation are accurate and submitted within established timelines.<\/li>\n<li>Exhibiting organizational time management skills, accountability, reliability, and punctuality.<\/li>\n<\/ul>\n<h6>RESPONSIBILITIES:<\/h6>\n<ul>\n<li>Reflecting and interpreting the Agency Vision, Mission and Core Values in his\/her own work with enthusiasm and commitment.<\/li>\n<li>Acting in accordance with relevant legislation and NCNW\u2019s policies, standards and procedures.<\/li>\n<li>Developing and maintaining respectful, cooperative working relationships to contribute to the integrated, seamless delivery of services to indigenous children, families and communities.<\/li>\n<li>Applying culture, values, traditions and teachings into programming where possible.<\/li>\n<li>Participating constructively in the supervision process with the immediate supervisor.<\/li>\n<\/ul>\n<h6>KNOWLEDGE, SKILLS &amp; ABILITIES:<\/h6>\n<ul>\n<li>Sound knowledge of the indigenous culture and the principle issues affecting the mental health of children, families and communities in the Niagara Region.<\/li>\n<li>Knowledge of at-risk youth-related issues, such as addictions, sexual orientation\/identity, peer\/social culture, human development, violence, teen pregnancy, gangs, human trafficking, etc.<\/li>\n<li>Knowledge of case management systems, reporting procedures and responsibilities and the ability to work within those systems.<\/li>\n<li>Effective written and oral communication skills, problem solving, advocacy and assessment skills.<\/li>\n<li>Ability to manage crisis and adverse situations with staff, youth and families.<\/li>\n<li>Ability to work within a multi-disciplinary team, as well as manage and maintain public inquiries.<\/li>\n<li>Knowledge and understanding of health and safety practices related to outreach work.<\/li>\n<li>Experience in harm reduction, safety planning and crisis intervention.<\/li>\n<li>Knowledge and understanding of the Child and Family Services Act, the Mental Health Act and other relevant legislation.<\/li>\n<\/ul>\n<h6>CONDITIONS OF EMPLOYMENT:<\/h6>\n<p>A. Must be willing to authorize a Vulnerable Sector Check and Driver\u2019s Abstract.<\/p>\n<p>B. Must possess a valid Standard First Aid\/CPR Certificate or be willing to obtain one.<\/p>\n<p>C. Ability to work flexible hours as determined by the Supervisor or designate in relation to service delivery requirements.<\/p>\n<p>D.
