The Health Worker will focus on improving health and diabetes prevention, early detection and control. They will also work on improving equitable access to care for Indigenous women and their families while fostering and promoting cross-cultural awareness and sensitivity for health practitioners. This position is funded through ONWA (Ontario Native Women\u2019s Association) and will connect with other Indigenous Women\u2019s Health Worker\u2019s around the province.<\/p>\n<h4>Service Provided Includes:<\/h4>\n<ul>\n<li>Complete one (1) diabetes awareness campaign \u201c13 Grandmother moon walks\u201d<\/li>\n<li>Complete minimum of four (4) regional activities (can be virtual)<\/li>\n<li>Complete minimum of twelve (12) workshops (can be virtual)<\/li>\n<li>Overall planning, coordination, implementation, and evaluation of the Indigenous Prevention Program funded by the Ministry of Health and Long-term Care\u2019s Ontario Aboriginal Diabetes Strategy.<\/li>\n<li>Develop a workplan that outlines the activities to fulfill the program deliverables and requirements.<\/li>\n<li>Develop, deliver and facilitate programming, workshops and presentations to promote diabetes awareness and prevention as outlined in the contribution agreement.<\/li>\n<\/ul>\n<h4>Qualifications:<\/h4>\n<ul>\n<li>A minimum college diploma in a health-related field and\/or equivalent knowledge and work experience in health promotion and diabetes health.<\/li>\n<li>Strong knowledge and experience of diabetes.<\/li>\n<li>Knowledge of not-for-profit organizations program delivery and experience working with Indigenous populations and communities.<\/li>\n<li>Must be dependable, able to follow instructions, respond to management direction and be able to improve performance through management feedback.<\/li>\n<li>Capacity to solve problems and to see different perspectives.<\/li>\n<li>Ability to plan, organize and manage time effectively.<\/li>\n<li>Highly motivated individual with the ability to work with minimal supervision.<\/li>\n<li>Excellent organizational and communication skills and ability to follow through on assigned duties.<\/li>\n<li>Ability to work independently to meet strict deadlines.<\/li>\n<li>Demonstrated ability to establish and maintain collaborative relationships with community members, indigenous partners &amp; mainstream agencies.<\/li>\n<li>Strong awareness of issues affecting Indigenous women and their families and awareness of available community resources.<\/li>\n<li>Good working knowledge of and established relationships with other service agencies, ability to network, create and maintain new relationships and partnerships.<\/li>\n<li>Experience coordinating workshops and\/or conferences.<\/li>\n<li>Ability and willingness to travel (when necessary) and able to work flexible hours, if needed.<\/li>\n<li>Proven ability to build trusting relationships at all levels and across all functions.<\/li>\n<li>Ability to effectively communicate across the organization.<\/li>\n<li>Creative, flexible, adaptable and change focused, and priorities and allocate time and resources.<\/li>\n<li>Ability to prioritize programs and activities.<\/li>\n<li>Proficient working knowledge of MS Office Software, internet and general office equipment.<\/li>\n<li>Reliable vehicle and appropriate vehicle insurance coverage and valid Ontario \u201cG\u201d class driver\u2019s license.<\/li>\n<li>Current, valid vulnerable sector criminal records check. Valid 1st Aid \/ CPR.<\/li>\n<li>Ability to speak an Indigenous language is an asset.<\/li>\n<li>Living, striving and practicing a healthy lifestyle.<\/li>\n<\/ul>\n<p><em><strong>*Note:<\/strong> This job description is not intended to be all-inclusive.
