{"id":172492,"date":"2025-12-02T13:22:17","date_gmt":"2025-12-02T18:22:17","guid":{"rendered":"https:\/\/www.jobgym.com\/jobboard\/?post_type=job_listing&#038;p=172492"},"modified":"2025-12-10T00:34:44","modified_gmt":"2025-12-10T05:34:44","slug":"outreach-worker-hospital-focus","status":"expired","type":"job_listing","link":"https:\/\/www.jobgym.com\/jobboard\/?post_type=job_listing&p=172492","title":{"rendered":"Outreach Worker- Hospital Focus"},"content":{"rendered":"<h3>Outreach Worker- Hospital Focus<\/h3>\n<h4>Gateway Residential and Community Support Services<\/h4>\n<p>&nbsp;<\/p>\n<p><strong>Wage:<\/strong> $25.59-$29.03 per hour<\/p>\n<p><strong>Hours:<\/strong> 0-35 per week<\/p>\n<p><strong>Requirements:<\/strong><\/p>\n<ul>\n<li>Community College Diploma or a University Degree in social services or other closely related field or a combination of education and experience which provides the required knowledge, skills and abilities as determined by the employer<\/li>\n<li>One-year experience working with people experiencing homelessness<\/li>\n<li>One-year experience in the community mental health field and\/or addictions<\/li>\n<li>A valid G drivers\u2019 license with a reliable vehicle and permission to carry rider attached with 2-million-dollar liability<\/li>\n<li>Valid CPR Certificate\/First Aid and Non-Violent Crisis Intervention Training<\/li>\n<li>Availability to work evenings and weekends<\/li>\n<li>Knowledge of homelessness service system and outreach community resources<\/li>\n<li>Good organizational, problem solving, time management and prioritizing skills<\/li>\n<li>Ability to work independently and as part of a team<\/li>\n<li>Excellent interpersonal skills with good oral and written skills<\/li>\n<li>A well-defined sense of diplomacy, including solid negotiation, conflict resolution, and people management skills<\/li>\n<li>Computer literacy, including effective working skills of HIFIS, MS Word, Excel, Teams, Outlook, and Outlook Calendar<\/li>\n<li>Understand and work within a person-centered approach, maintaining political and cultural sensitivity<\/li>\n<li>Strong morals and ethics, along with a commitment to staff and client privacy<\/li>\n<li>Demonstrated ability to function within and contribute to a fast-paced team environment<\/li>\n<li>Demonstrated commitment to ongoing professional education<\/li>\n<li>Physically able to perform all assigned tasks<\/li>\n<li>Ability to work under minimal supervision<\/li>\n<li>Bi-lingual in both French and English is an asset<\/li>\n<\/ul>\n<p><strong>Police Clearance and Vulnerable Sector Check Requirements:<\/strong><\/p>\n<ul>\n<li>To ensure the safety of the individuals we serve, all successful candidates for this position will be required to provide a Police Clearance and Vulnerable Sector Check from their local police service. A clear Vulnerable Sector Check is a condition of employment. Any prior convictions or offenses (outside the scope of the Vulnerable Sector Check) will be assessed in accordance with applicable privacy and human rights legislation. A criminal record may be considered in the hiring decision, in accordance with the Ontario Human Rights Code. Applicants will have the opportunity to discuss any relevant information once the check is requested.<\/li>\n<\/ul>\n<p><strong>Duties:<\/strong><\/p>\n<ul>\n<li>Hospital Focused:\n<ul>\n<li>Check in with the Emergency Department Charge Nurse upon arrival<\/li>\n<li>Provide support to clients who are unhoused and presenting at the Emergency Department, including:<\/li>\n<li>Connecting to shelter placements<\/li>\n<li>Completing VISPDAT assessments<\/li>\n<li>Conducting ongoing assessment for program eligibility<\/li>\n<li>Facilitating connection to NASO services for next-day follow-up<\/li>\n<li>Complete HIFIS documentation for all clients served<\/li>\n<\/ul>\n<\/li>\n<li>General Outreach:\n<ul>\n<li>Engaging people experiencing unsheltered homelessness at the hospital<\/li>\n<li>Provide meaningful, creative engagement strategies to build rapport<\/li>\n<li>Assessment (including risk, needs, goals, and strengths)<\/li>\n<li>Case planning, implementation, evaluation, and review<\/li>\n<li>Referrals to appropriate accommodation resources and housing providers<\/li>\n<li>Facilitating access to physical and mental health support services<\/li>\n<li>Consideration for cultural and spiritual well-being<\/li>\n<li>Work in collaboration with the Niagara Assertive Outreach Team and the hospital<\/li>\n<li>Collaborate with substance-use harm reduction services, healthcare providers, and the criminal justice system<\/li>\n<li>Collaboration towards social inclusion<\/li>\n<li>Person centered advocacy<\/li>\n<li>Reconnecting people with family, friends, and social support<\/li>\n<li>Ensuring the client is informed of the Housing First principles<\/li>\n<li>Attend and participate in all team\/staff meetings or training as required<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><em>This position description reflects the general duties of the position, but it is not intended to be a detailed description of all duties that may be inherent in the position.
