Contract Activation Therapist

Job Description

  • Plan and implement programs with the resident and/or their Power of Attorney (POA)/Family member, documenting their purpose, goals, and objectives.
  • Provide assistance and adaptations to facilitate residents’ participation in activities.
  • Plan activities and trips outside the facility.
  • Provide individualized and group therapy services based on assessed need of the resident(s) including therapy sessions for residents not interested/unable to participate in larger groups.
  • Ensure therapy interventions are aligned with resident’s nursing and personal care activities.
  • Ensure the activation section of new admissions are completed within twenty-one (21) days from the admission date.
  • Assist Physiotherapists with residents’ exercises programs.
  • Participate in staff meetings, committees, and in-service education designated by the Director of Care.
  • Participate in Resident Care Conferences, as required by the Director of Care.
  • Develop and maintain up-to-date Resident Care Plans and RAI MDS (Activation section) as delegated by Director of Care.
  • Support Registered Staff by providing support, companionship and comfort care to palliative residents.
  • Under the direction of the Volunteer Coordinator, the Activation Therapist assists with the social events for the Village Residents. (This involves five (5) hours per week among the staff of the Activation Department)
  • Oversee volunteers and students who provide support to recreation events by following organization volunteers program policies and procedures.
  • Coordinate the Pet Therapy Program for Long Term Care Residents, including compliance on Infection Control Protocols and Ministry requirements.
  • Prepare monthly reports as required by Director of Care.
  • Perform additional duties, as required

WHAT WE ARE LOOKING FOR

  • Three (3) to five (5) years of experience in a Long-Term Care Facility.
  • Ability to communicate clearly and sensitively with vulnerable people and their families.
  • Demonstrated ability to work independently and as part of a team.
  • An organised, efficient and flexible approach to the job, as well as good problem-solving abilities.
  • Time management skills and the ability to meet the needs of several clients at one time
  • A high level of patience, compassion and emotional resilience are “essential”
  • We are looking for somebody who able to demonstrate character traits such as empathy, flexibility and the ability to provide warm encouragement.

Please note that successful applicants will require a current Police Reference Check suitable for working with the vulnerable sector prior to commencing employment. It is required that all new hires are to have first and second dose of tuberculosis (TB) vaccination as a condition of employment with Niagara Ina Grafton Gage Village.

A Diploma in Recreation/Leisure studies from a community college or university.
